Create a databased form opportunity

Here's an offer for anyone interested. I need a form created for my site and have given up trying to make it work. It's a databased module for phpnuke atleast 7.9.
I will paypal $20 to the first person that can give me the finished product.

• The link to the input page should either be by a link that only admins can see underneath the result page or by going to the admin.php page as one of the icons.

• The input page should have the fields to the left with the description on the right of it. All inputs should be text fields except for the biography field which should be a large text field about the size of a wysiwyg box or close to it. The field itself does not have to be actual wysiwyg. And the picture field should be a url upload text field. The upload field should be by browse the harddrive only, not by url.

here is the list of the input fields: 
last name (or single name)
first name
birth name
death year
belief
biography
picture upload

• the lastname or single name input should be a link that leads to the biography page.

The result of the database page is the following:

Lastname    Firstname    Birthyear    Deathyear    Belief
Smith       John         1853         1893         Anglican
Jones       Roger        1689         1737         Puritan

The column words should be linkable making the whole list order according to alphnumeric order.
Lastname and firstname should be alphabetical linkable(you click on it to make it alphabetically ordered)
brithyear and deathyear should be numeric linkable(youclick on it to make it numerically ordered)
belief is alphabetical order linkable in the same way.

The entire result list should be able to handle atleast 250 names.

When clicking on the lastname (or singlename) of the person, the link should take you to a biography page.

• The biography page should have the photograph top center and then the first and last name in <h3> tags and then two <br> before the start of the biography text that follows.

• The bottom of the biography page should contain a link to the database page, like: Return to Name List Page
