My own recipe usually includes:

1) Try to reasonably separate markup of php code, aka use a templates.
2) Use functions & classes to handle non trivial tasks.
3) Use exceptions for error handling, instead of die("error blah blah"), it will pinpoint where exactly the error is, even in production.
4) In productions, configure the server to dump errors to a file instead to the navigator.
5) Use includes with relative paths so moving your code from local to the server is "upload & play".
