How do you stop people from using proxy servers to access a website ?

dogma: can you clarify please? Are you a network admin and would like to stop people from using a proxy server to get to a website you have blocked from your network? Or are you talking about a website that you own, and you want to block people from getting to your own website via a proxy server?

You can "deny" connections to specific hosts or ip addresses in your .htaccess file if you have a list of ip addresses. I don't think you would have an all encompassing list so I doubt you can get 100% coverage against all possible proxy servers out there. Someone with more knowledge please correct me if I'm wrong.
