My site receives about 10,000 uniques and about 50,000 page views a day. How much should I charge for CPM or a monthly rate for a 728 x 60 banner advert?

This is reasonable, I think...
10,000 Impressions for $10.00 CPM/ $1.00

You can discount the CPM rate for those who sign a long term commitment.
