Here is the code for the form I created. It seems ok but I don't know how to make it create a database. I only know html. The name of this file is index.php and it's located under the root at /modules/Grievance . Thank you all.

I got an error saying this code is too big to post on this forum, sorry.

Any body know how I can do this, please? Please help.
